Biologics I tried: Humira, Rinvoq, Simponi and now on Taltz. I don’t think I’ve been on any IL 12/23 biologics. When I first saw my new rheumy I was off Rinvoq and just a few months into Simponi and in the gap my psoriasis flared. That became the focal point so she switched me to Taltz after six months of Simponi. Taltz was better on my psoriasis but not perfect. Still battling painful spots on my eyelids that forced me to use Protopic tacrolimus topical…which I hate because of side effects and don’t like using perpetually around my eyes. BTW…my longest misery in the gap was while waiting for Rinvoq to work which put me on prednisone crap that was horrible to wean off of. Rinvoq was definitely the slowest to get going. I thought I was on a placebo.

Well I am glad to read that you are getting off Rinvoq. I was on Cosentyx for a few years and everything Poo said is true. It takes a long time to “load” but after that I felt great, until it stopped working. I hope that is not the case for you and it works for years and years!
Oddly only Rinvoq in the JAK inhibitor class is useful for us. The rest cause too many other issues. My rheumy doesn’t offer Xjelanz (if I’ve spelt that correctly). She said the vast majority got blood clots but she does offer Rinvoq. However there is research going on with a new one called Tidrakizumb and then Icotrokin which is an tablet form of peptide to block IL23. So thankfully our research into new meds for us continues.

Oh and another thing. Cosentyx lasts in your body for between 110 and 155 days. So things like pausing shots for antibiotics etc truly makes little sense. It does need cogent pausing for surgery though so talk that through beforehand. However it hates having the dosing schedule messed up in the first year so try not have to have it messed up by anything kind of voluntary during that time period if you possibly can.
And remember if it doesn’t like you for whatever reason, you have a long washout period before starting the next one. I waited four months before starting Tremfya for that very reason. Our bodies don’t really like having two biologics at once. And sadly for a friend of mine, she wasn’t counselled to wait more than the usual 6 weeks between her last Cosentyx shot and starting a new biologic and then developed a serious skin issue as a result, which took a year on low dose steroids only to clear so PsA treatment was halted for all that time. That is a really rare occurrence though, but it was still one I wasn’t inclined to risk.
